Becoming a Member

If you don’t have health insurance, can’t afford the cost of commercial coverage and are not coved by Medicaid or Medicare, you may be eligible for CHS enrollment.  To become a CHS member, individuals must meet eligibility requirements (listed below) and submit a Membership Application form for review.  The application is comprehensive and must be completed fully with all required documentation in order to be considered for enrollment process.

Who is eligible?
To be eligible for CHS membership, you must meet the following criteria:

  • Must be a resident of Atkinson, Chester, Danville Derry, Hampstead,  Londonderry, Plaistow, Salem, Sandown or Windham, New Hampshire.
  • Must meet specific income and asset guidelines (download Application Form).
  • Not be covered by health insurance of any kind (download Insurance Verification Form).
  • Must not have an open Workers’ Compensation claim.
  • Must not be eligible for affordable insurance through an employer.
  • Our staff is available to answer any questions pertaining to the application and will be happy to meet with individuals who have difficulty completing the application on their own.

How long does the application process take?
Evaluation of the application is generally completed within 10 days of submission and all applicants will be notified by CHS whether they are accepted for enrollment or not.

What does CHS Cost?
CHS is not a free care program. Our philosophy emphasizes that members share a fair portion of the responsibility for the expense of  their care. CHS members are required to make reasonable co-payments for medical office visits and for prescription medication at the time of service. Charges for other health services, such as dental care, are determined based on income guidelines for various procedures.
