CHS Has Moved! 
We are pleased to announce that CHS has moved into our new office. We are now located at 14 Tsienneto Road, Suite 301, Derry, in the Medical Office Building adjacent to the Goddard School.

Helpful Reminder for Members

You are responsible for having your membership card with you at all times and for making co-payments at every scheduled visit.

Once enrolled, make it a habit to save documentation. Place an envelope in a prominent location in your home. Deposit paycheck stubs, tax returns, bank statements and other important documents. Remember to include receipts for any medical payments you have made in the event that there is a dispute about a balance. This will make the recertification process much easier for you.

Recertification packets are typically due in to CHS on the first Friday of the month in which your card expires.  For instance, if your current membership card expires on April 30, 2012, your recert packet will be due on or before April 6, 2012 to give us ample time for review.

No Friday Pharmacy

Please remember that until further notice the CHS pharmacy is closed on Fridays. If you receive emergency prescriptions on a Friday or need a prescription refilled for the weekend, please contact our office to get information about alternatives for filling the prescription.  Call CHS at 603-425-2545.


Generic Meds Can Save You $

Remember when you go to your doctor visits to ask if there are generic medications that you can take instead of name brands. Walmart, Target and other retail stores now offer many  $4 generic medications. It could end up saving you some money!


CHS Dental Program

The CHS Dental Program is for members of the Health Care Program only. The Dental Program Coordinator is available on Monday, Tuesday, and Thursday between 8:30 a.m. and 4:00 p.m. If you have questions or need an appointment, please call the office at 603-425-2545.
