Member Services
What happens once you are enrolled?
Once enrolled, new members must attend an Intake Appointment with their Health Care Manager (HCM). At the intake appointment, a Health Risk Assessment is completed and reviewed with the member; the program, policies and membership obligations are explained; a Member Agreement is signed; and the enrollee is assigned to a Primary Care Physician within the network.
CHS membership is valid for six months at a time. Individuals may remain in CHS on an ongoing basis, but eligibility must be reviewed and verified every six months to continue membership.
What Services does CHS provide?
CHS members have access to a wide range of health care and prevention services, including:
- Office visits with a Primary Care Physician
- Referrals to Medical Specialists when needed
- Dental Care Services
- Prescription Medications
- Diagnostic Tests
- Behavioral Health Care Services
- Hospital-based Services
- Community Support Referrals
Case Management
CHS offers case management support services to members as well as any resident of the service area who needs assistance in identifying resources for which they may be eligible, including services outside of CHS.
CHS Office Hours
CHS is located at 41 Birch Street in Derry, NH. Our office hours are Monday-Friday, from 8:30 a.m. to 4:00 pm.
Onsite Pharmacy Hours
CHS has a fully-licensed onsite pharmacy that is available to dispense prescription medications and refills for CHS members. The Pharmacy is currently open: Monday and Wednesday, 1:00 to 4:30 p.m. - Tuesday and Thursday, 9:00 to 11:00 a.m.
